
That satisfying crack during a chiropractic adjustment is one of the most recognizable sounds in healthcare, and one of the most misunderstood. The good news: it is not your bones breaking, grinding, or even touching each other.
Every moveable joint in your body, from your spine to your knees, knuckles and hips, is enclosed in a sealed, fluid-filled capsule. That fluid is called synovial fluid, and its job is to lubricate and nourish the joint so it moves smoothly and without friction.
Dissolved within that synovial fluid are gases, primarily carbon dioxide, oxygen and nitrogen. Under normal conditions these gases stay dissolved quietly in the fluid, much like the carbonation in an unopened soda bottle.
When your chiropractor applies a quick, controlled force to a joint, a technique called a spinal manipulation or high-velocity low-amplitude thrust, the joint capsule is rapidly stretched. That sudden stretch causes a sharp drop in pressure inside the fluid.
The pressure drop is the key. When pressure falls fast enough, the dissolved gases can no longer stay dissolved. They come out of solution and a gas cavity opens up inside the joint space. This process is called cavitation.
The pop happens at the instant the joint surfaces separate and that gas cavity appears. Researchers still debate the last detail, whether the sound is the cavity forming or partly collapsing a moment later, but they agree on what is making it: gas in the fluid, not bone.
In short: the sound comes from a gas bubble inside your joint fluid. It has nothing to do with bones cracking, snapping, or grinding against one another.

The relief people feel after an adjustment is not just the satisfying sound. When the joint capsule is rapidly stretched, it stimulates mechanoreceptors, the specialized nerve endings in joint tissue that detect movement and pressure.
That nerve stimulation can dampen pain signals coming from the area and prompt the surrounding muscles to relax. The result is less tension, more mobility, and often a real reduction in discomfort.
Adjustments do produce real, measurable physical change: the small facet joints at the back of the spine open up slightly, and the spine measures as less stiff immediately afterward. Exactly how that adds up to feeling better is still being studied, and honest answers in this field say so.
